Create a PDF document, don't use MS Word or other formatted files. You can create in MS Word if you wish and then convert to PDF.

I usually just put a notepad or word file on the C.D. with the relevant
information i use notepad because it gives plain text which is readable by
most systems. Just put whatever is relevant to the show/work/your career in
the resume....not too many pages. Put that in a separate, clearly titled
file as well.
