It's a Phillips 18" 15 watt black light.

Sorry, we can't give you the part number right now, the stock boy is out
in the yard....

(I can't swear to the price, but I'm pretty sure that's all I paid. It
was a year ago, and I've used a lot of amidol in my day...)
