Hi Saffron - glad you found what you were looking for!  The book is a
Blandford Press publication, ISBN 0-7173-1652-5.

The example photo I posted is from the book.  The tech details say that it
was taken on a Pentax 6x7, 1/250th sec at f4.0, HP5 rated at 1600 and
developed in ID11.

He used tungsten lamps, not flash, and the light background has to be more
brightly lit than the model in order for the 'sylph' effect to take place -
if the background is much darker the reverse happens, and the model 'melts'
into the background producing a fattening effect!
